  • GET STARTED print out our User Agreement and if applicable Drug Screen Setup Form (these document can be filled out online then printed, or printed then completed manually). Fax (888-763-7366) the completed User Agreement to GIN. After we receive your completed user agreement your company will be set up in our system and any necessary forms will be emailed to you.
